Monthly Installment Plan Agreement and Consent *

I consent and agree to the below terms and conditions

1. I promise to submit payments of a minimum of $200 on a monthly basis until the full term's tuition is payed for.

2. I promise to complete payment of tuition due ($800) for the first term by January 15, 2020.

3. I consent Zidni Islamic Institute to charge me (via PayPal or Stripe) the total amount as indicated under "Total Amount". I understand and agree to the terms of refund: last day to drop and request refund for first term tuition or amount paid is October 15, 2019.

4. If the student has any difficulty in making payments, they should contact admissions@zidniinstitute.org so that payment arrangements can be made.
